Method 1:

1. Click on the mail icon in the top right of eCampus
2. Click on the blue Inbox button
3. Click on the Apps button in the Outlook Web App (it may be on the top left or right
4. Click on All apps
5. Click on Forms

Method 2:

1. Navigate to https://forms.office.com/
2. Click Sign in in the top right
3. Enter your University of Phoenix email address: username@email.phoenix.edu
4. Click Next
5. Then enter your University of Phoenix username and password
6. Click Login

Note: The license for Microsoft Forms has a 24 hour replication time. Students with licensing that has not replicated may see the error "Your account is not enabled for Microsoft Forms" after following the steps in this article.
