Question:
How do I access the Health Insurance Election Form?
Answer:
To access the Health Insurance Election Form, perform the following:
  1. Click the Academic Plan link
  2. Click the Program tab
  3. Click Health Insurance Election Form, under Services
    Note: If you are not required to provide a Student Health Insurance Acknowledgement, the link will not appear
  4. Click link to access form
  5. Complete according to instructions on form
  6. Click Home to return to eCampus Home page
