Completing a Family Educational Rights & Privacy Act (FERPA) Release Form in the Academic Plan (Program Tab)

Question:
How do I complete the FERPA release form in the Academic Plan (Program Tab)?
Answer:
To complete the FERPA release, perform the following:
  1. Click the Academic Plan link
  2. Click the Program tab
  3. Under Services, click Education Record Release (FERPA)
  4. Click Yes to the question asking whether you would like to release your educational records
  5. Click Next to continue to the next step
  6. Select the box next to the question asking whether you would like to release your educational records to a third party
  7. Add the information, as requested
  8. Select the method to use to release this information:
    • By Mail
    • By Fax
    • By Phone/In-Person
  9. Click Next
  10. Click FERPA Release Form link and verify the information is correct in the Portable Document Format (PDF)
    Note:You may print the PDF for your own records.
  11. Close the PDF window or tab
  12. Click Next
  13. Check the boxes to Consent to Electronic Signature Content and to acknowledge the Hardware and Software Agreement
  14. Click Next
  15. Check the box to accept the Terms and Conditions
  16. Input your First Name and Last Name and your Password (eCampus password)
  17. Click Submit FERPA Release Request
  18. You will receive a message:
    Your FERPA release required has been received by the University
