Locating Leave of Absence (LOA) Application

Question:
Where can I locate the LOA application on the Student and Faculty Portal?
Answer:
To locate the form, please follow the steps below:
  1. Log in to ecampus.phoenix.edu
  2. Click the Program tab
  3. Click Leave of Absence under Services
    Leave of Absence
  4. Select one (1) of the following:
    • Click Request or Change Leave of Absence to file out the Leave of Absence application
      Note: Follow the steps included on the form to request, extend, or change a LOA.
    • Click My Leave of Absence History to access a list of your Leave of Absence Requests
    • Click Leave of Absence Policy to download the Leave of Absence Policy PDF

Note: You must open and review the Consumer Information Guide before you will be able to check the acknowledgement, I have read and understand the University Leave of Absence Policy, in the guide. Additionally, the Additional Options section provides information about dropping a course or withdrawing from the University of Phoenix. Please contact your Financial Advisor or Academic Representative for questions about either option or when filling out the LOA.
