To submit a name change or make a correction with your name, please complete the following:
Students currently enrolled:
Note: Students can complete a name change directly with the Office of Admissions and Records if they are not currently enrolled at the University and not applying for a new program. If the student is currently enrolled, or the student is applying for a new degree, their Academic Counselor should assist with the name change process.Faculty:
