Question:
Who do I need to contact concerning a name change or correction?
Answer:
To submit a name change or make a correction with your name, please complete the following:

Students currently enrolled:
  • Contact your Academic Counselor
  • Name change forms are sent to students via Adobe Sign
Students not currently enrolled:
  • Contact the Office of Admissions and Evaluations (OAE)
    • Toll-Free: 1-800-866-3919
    • Local: 1-480-446-4600
    • Monday - Friday: 6:00 a.m. - 5:00 p.m. (MST)
Note: Students can complete a name change directly with the Office of Admissions and Records if they are not currently enrolled at the University and not applying for a new program. If the student is currently enrolled, or the student is applying for a new degree, their Academic Counselor should assist with the name change process.
